A man in his 30s with hypertension presented with epistaxis. He also reported a week-long history of fever and joint pain. Three days after the onset of the fever, he developed nasal pigmentation that progressively worsened over the following week. His platelet count was normal. Clinical examination revealed brownish pigmentation primarily affecting the dorsum of his nose [Figure 1]. The dermoscopic evaluation demonstrated an exaggerated pseudoreticular network with dilated follicular openings, raising suspicion for the early chik sign [Figure 2]. A positive immunoglobulin M chikungunya serology confirmed the diagnosis of chikungunya fever.
